Authorization and Disclosure Statement & Applicant Declaration

I affirm that all the information provided in this rental application is true, complete, and accurate to the best of my knowledge. I understand that this application is subject to the approval of the Landlord, Association, or agent of the Owner.

I hereby authorize Florida HomeGroup Realty LLC, as the authorized agent of the Owner, Association, or Landlord, to verify all the information contained in this application, including but not limited to employment, income, rental history, banking references, personal references, credit history, and criminal background. I understand that an investigative consumer report may be obtained, which may include information related to my character, creditworthiness, general reputation, personal characteristics, lifestyle, and any public records including felony or misdemeanor charges, evictions, and bankruptcies.

I further authorize Florida HomeGroup Realty LLC to disclose the results of any such reports to the property owner and their licensed representative for the sole purpose of evaluating my ability to perform under the terms of a lease or rental agreement. I understand that all information obtained will be kept confidential and used strictly for tenant screening purposes in accordance with the Fair Credit Reporting Act (FCRA).

I acknowledge that the application fee is non-refundable under any circumstances once the application has been processed, regardless of the outcome. I also understand that a valid government-issued photo ID must accompany this application.

I also understand that any false or misleading statements made in this application may result in denial, termination of lease, or legal consequences.
